What programs or services does your nonprofit offer?
– Nurse-Family Partnership is a free home visitation program in which nurses mentor income-eligible first-time mothers and provide education and support from early pregnancy through the child’s second birthday.
– The Women, Infants, & Children nutrition program provides funding for nutritious food and education for mothers, infants, and children under 5.
Early Intervention services are available to children from birth through age three. They help them learn, grow, and reach developmental milestones wherever they are, whether it is at home, in the neighborhood, or in day care.
Pediatric Home Connection offers shift-duty care for children with complex medical needs. This includes tube feedings and ventilators.
– The Healing Patch Children’s Grief Program offers free services after the death of a loved one. Services include peer support sessions in Altoona or Ebensburg as well as in-school support groups.
Private Duty Home Care offers personal care, household management and companionship to clients who wish to remain at home.
– The AIDS Intervention Project is a community-based, confidential service that provides case management, education, and outreach activities for HIV/AIDS patients.
UPMC Home Healthcare helps patients recover from illness, injury, or surgery. Short-term skilled services are provided for patients of all ages under a care plan approved by the patient’s physician.
Family Hospice provides support for patients with a goal of quality of their lives. The focus of care is on pain management and symptom management so that the patient and their family can live life to its fullest.
Upcoming events: People can donate to the Lights of Love holiday giving campaign. “shine a special light”In memory or honor of someone you love. All proceeds will benefit Family Hospice and Grief Services. A visual tribute with lights at the Sheraton Drive office in Altoona is provided from Nov. 22 to Jan. 7. Family Hospice is represented by white lights, while grief services for children and families are represented by blue lights.
